The following form is for students who are currently enrolled in another Clark County School, but are NEW to Shadow Ridge for the 2020/2021 school year.

This form is not for students who are new to the Clark County School District, or if you previously attended a Clark County school, but are not attending one currently. New students will need to use the form "2020/2021 New Student Registration" also located below.

2020/2021 New Student Registration

 

This Registration Form is for students who are not currently enrolled in a Clark County School District School.

If you are currently enrolled in a Clark County School District school, but are new to Shadow Ridge this year, use the form "2020/2021 Current CCSD Student Registration" located above.

2.  Come to the Registrar's Office at Shadow Ridge High School.  Please come with the following documentation: (While Schools are closed, please attach the documents in the indicated places on the registration forms.)

 

Immunization Requirements:

  • DPT/DT - 4 required - 4th dose must be on or after 4th birthday

  • Tdap - required except if less than two years since last DPT shot

  • Polio - 4 required - 4th dose must be on or after 4th birthday

  • MMR - 2 required - 1st dose on or after 1st birthday, 2nd dose at least 4 weeks apart

  • MCV4 (Meningitis) - 1 required - on or after age 10 years of age (effective for all students enrolling in the 2017-2018 school year who are new to the state of Nevada).

 

 

New students to Nevada are also required to have the following immunizations (returning CCSD students are encouraged to receive these immunizations):

 

  • Hepatitis A - 2 required, separated by 6 months or doctor verification of disease.

  • Hepatitis B - 3 required, total series covers 16 weeks or doctor verification of disease

  • Varicella (chicken pox) - under 13 years of age, 1 required; over 13 years of age, 2 required separated by 2 months or doctor verification of disease

 

 

Birth Certificate

  • ‚ÄčOriginal birth certificate

 

Proof of Residence

  • Must present any one of the following: a utility bill (water, gas, power or garbage) or a rental/purchase agreement. Document submitted MUST be current and in the parent's name. Telephone and cell phone bills are not an acceptable form of proof of residence.

 

 

Affidavit of Residence

  • Residential Affidavit:  Only required if family is "living with others" and is unable to provide proof of residence in parent(s) name. Residential Affidavit must be notarized and proof of address from home owner and drivers license of homeowner must also be provided with Residential Affidavit at time of enrollment.  

 

 

Guardianship Documentation

  • Only required (when applicable) if student is living with someone other than parent. Students in this situation may not enroll in classes until this documentation is provided.

 

 

Academic Records

  • If new to the CCSD, an unofficial transcript (preferred), most recent report card or withdrawal documentation from previous school is acceptable.

 

 

Zone Variances

Zone Variance applications are submitted in person starting March 1st thru April 30th, 2020.  New to Shadow Ridge students must submit an Unofficial transcript, Attendance and Behavior Reports from their current school at the time of application.  If the student is accepted, further documentation will be required upon enrollment.  Students currently attending Shadow Ridge on a Zone Variance may submit Zone Variance applications between the dates of March 1, and April 30, 2020.  All approved Zone Variances will require proof of address and additional documentation to be supplied at a later date.  Thank you.
